FIP Blood Test Results in Cats

Feline infectious peritonitis (FIP) is a serious, often fatal disease caused by a mutation of feline coronavirus inside the cat’s body. Because the disease can affect multiple organs and present in wet, dry, or mixed forms, FIP blood test results in cats are rarely diagnostic by themselves. Instead, veterinarians use bloodwork as part of a broader clinical picture that includes symptoms, imaging, fluid analysis, and sometimes molecular testing. Understanding the patterns seen in FIP blood tests can help cat owners and veterinary professionals recognize suspicion earlier and move toward faster confirmation and treatment.
One of the most important points in FIP diagnosis is that there is no single routine blood test that proves FIP with complete certainty. Bloodwork can show changes that strongly support the disease, especially when combined with fever that does not respond to antibiotics, weight loss, poor appetite, abdominal swelling, breathing difficulty, or neurologic and ocular signs. The value of blood tests lies in identifying inflammation, immune system imbalance, organ involvement, and disease severity.
Common Blood Test Patterns Seen in FIP
A complete blood count and serum biochemistry panel often reveal several abnormalities in cats with FIP. The most recognized pattern is a strong inflammatory response with increased globulins and decreased albumin. This creates a low albumin-to-globulin ratio, which is one of the most useful laboratory clues. A ratio below about 0.8 raises concern, and a ratio below 0.4 is more suggestive of FIP when paired with compatible clinical signs.
Hyperglobulinemia reflects an overactive immune response, while hypoalbuminemia may occur because of inflammation, liver dysfunction, protein loss, or reduced production. In many cats, total protein is elevated because globulins rise more than albumin falls. Some cats, however, may have normal total protein early in disease or if they are dehydrated, so a normal protein level does not rule out FIP.
Another frequent finding is a mild to moderate non-regenerative anemia. This happens because chronic inflammation suppresses red blood cell production. Leukocyte changes can vary, but many cats show neutrophilia, lymphopenia, or both. These changes are not specific to FIP, yet they contribute to the overall inflammatory profile.
Important Serum Chemistry Findings
Biochemistry results often reveal additional clues. Bilirubin may be elevated in some cats, particularly when the liver or bile flow is affected. Liver enzyme elevations can occur, although they are not consistent in every case. Some cats develop electrolyte disturbances or increased acute phase proteins. These findings support systemic disease but still need clinical correlation.
The acute phase protein serum amyloid A is especially useful in some settings. Markedly increased serum amyloid A suggests active inflammation and is commonly seen in FIP. It is not unique to FIP, but a very high level in a cat with compatible signs can strengthen suspicion. Other inflammatory markers such as alpha-1-acid glycoprotein may also be elevated and are often discussed in FIP workups.
If kidney values are altered, clinicians must consider whether dehydration, systemic inflammation, or direct organ involvement is present. In advanced disease, multiple organ systems may be affected. Blood tests help determine how widespread the process has become and guide supportive care decisions.
Albumin-to-Globulin Ratio and Total Protein
The albumin-to-globulin ratio deserves special attention because it is one of the most discussed blood test results in FIP. Cats with FIP often show high globulins due to chronic immune stimulation and low albumin due to inflammation or protein loss. The resulting ratio is more informative than either value alone.
Veterinarians often interpret this ratio together with total protein. For example, a cat with fever, weight loss, and effusion may have total protein above the reference range, globulins elevated, albumin reduced, and an albumin-to-globulin ratio strongly suggesting FIP. Still, other diseases such as lymphoma, chronic bacterial infection, and immune-mediated disorders can produce similar results. Bloodwork points the clinician in the right direction, but it does not replace definitive diagnosis.
Blood Test Results in Wet FIP and Dry FIP
Wet FIP, also called effusive FIP, often produces more dramatic laboratory findings. Cats with abdominal or chest fluid may show marked hyperglobulinemia, low albumin, and a very low albumin-to-globulin ratio. In addition, effusion fluid usually contains high protein and a characteristic inflammatory profile. Blood tests in wet FIP may look more obviously abnormal because the disease progresses rapidly and causes intense systemic inflammation.
Dry FIP, or noneffusive FIP, can be harder to detect from bloodwork alone. These cats may have subtle or inconsistent abnormalities, especially early in disease. Some present mainly with fever, poor appetite, lethargy, weight loss, eye disease, or neurologic signs. Their blood tests may show only mild changes in globulins, albumin, or white blood cell counts. Because dry FIP is less obvious, the full clinical picture becomes even more important.
FIP and Organ-Specific Laboratory Clues
When FIP affects the eyes or nervous system, blood test abnormalities may still be present but often do not reflect the true severity of organ involvement. Uveitis, retinal changes, seizures, ataxia, or behavior changes can occur even when standard lab values are only moderately abnormal. For this reason, normal or near-normal bloodwork cannot exclude FIP in a cat with neurologic or ocular signs.
Some cats show changes consistent with vasculitis, systemic inflammation, or immune-complex disease. These effects help explain why FIP can damage many tissues, including the liver, kidneys, eyes, central nervous system, and abdominal organs. Because the disease is multisystemic, a single blood result rarely captures the complete picture.

How Veterinarians Use Blood Tests for Diagnosis
A veterinarian may suspect FIP when bloodwork shows a combination of low albumin, high globulins, a low albumin-to-globulin ratio, anemia, and inflammatory changes in a cat with persistent fever or effusion. However, diagnosis usually becomes stronger when these findings match imaging, cytology, and fluid analysis.
For wet FIP, analysis of abdominal or chest fluid can be extremely helpful. PCR testing on effusion or tissue samples can add value, but results must be interpreted carefully because feline coronavirus infection alone does not equal FIP.
In dry FIP, advanced diagnostics may include imaging, ophthalmic examination, neurological assessment, and tissue sampling when possible. Bloodwork remains important because it tracks inflammation and may help monitor response during treatment.
Monitoring Treatment Response with Blood Work
Blood test results are also valuable during treatment and follow-up. If a cat receives antiviral therapy, supportive care, or both, veterinarians often monitor albumin, globulin, hematocrit, white blood cells, bilirubin, and inflammatory markers over time. Improvement in the albumin-to-globulin ratio and normalization of inflammatory values can indicate a favorable response.
Cats that recover from FIP often show gradual improvement in appetite, energy, body weight, and laboratory values. Persistent abnormalities may suggest incomplete response, relapse, or another concurrent disease. Repeated blood testing allows the clinician to adjust therapy and assess whether the cat is moving in the right direction.
Limits of Blood Testing in FIP
Despite their usefulness, FIP blood test results have clear limitations. Many laboratory abnormalities are not specific to FIP and can appear in lymphoma, liver disease, bacterial infection, chronic inflammatory disease, and other systemic disorders. Some cats with confirmed FIP may have only mild abnormalities early on. For that reason, no blood panel should be read in isolation.
The most practical approach is to combine laboratory findings with history, physical examination, imaging, effusion analysis, and targeted testing. When all data point in the same direction, the probability of FIP becomes much higher. Early recognition matters because cats that begin appropriate management sooner may have a better chance of clinical stabilization.
